Our Geriatric Care Managers perform the following tasks:

• Assess the physical, cognitive, emotional and environmental needs of the client
• Observe, listen and create a plan of care with the client and family
• Do a safety and elopement review of the home and make recommendations for hardware and technology.
• Plan and implement a review of medical condition, including a prescription and supplement review by a pharmacist
• Coordinate medication set up with prescription monitoring
• Establish schedules for hygiene, exercise, and activities
• Follow-up and reassess for maximum client satisfaction

Whether you or your loved one need assistance around the clock or for just 4 hours a day, our home care services include:
